Palliative Care Co-ordinators
What is the Service?
This is a nurse led service which facilitates the discharge home from acute care or a local hospice. They work with the community services to provide patient care within a home environment. The service also provides palliative care education in care homes.
Aim of the Service?
The aim of the in reach service is to enable a safe, smooth and seamless transition of care from hospital to community. This is for patients whose preferred place of care is either at home or within a care home.
Service Referral
Referrals are accepted from patients, carers and health and social care professionals. Referrals can be made through the CHCP CIC single point of access for Specialist Palliative Care Services by telephoning 01482 247111.
Referrals will only be accepted if the patient meets the following criteria:
- Patient must be over 18 years old
- Registered with a Hull GP
- Have a medically idenitfied palliative care need
Expected Outcomes for the Service
To ensure that patients with a life limiting illness receive the highest possible level of care and have a smooth transition of care from secondary care.
