Wellbeing Primary Care Liaison Service
The Wellbeing Primary Care Liaison Service works in partnership with GPs and hospitals, voluntary organisations and health promotion teams to promote annual health checks for people who have learning disabilities, severe mental illness and autism in Hull.
Alongside this we focus on ensuring that CHCP services adhere to the Accessible Information Standard and that patient's health needs are met by identifying and putting in place reasonable adjustments. We provide a support, advice and guidance service to address health inequalities and ensure fair access to CHCP health services.
At the heart of our service, we identify patients needs and promote the uptake of annual health checks. We do this by delivering training packages, creating accessible resources (easy read). And being a source of support advice and guidance to professionals.
