
Community Stroke Team
Hull & East Riding Community Stroke Service
Following your referral to the Hull & East Riding Community Stroke Service, you will have an appointment with a member of the team who will go through a screening questionnaire with you. This will give us information about the stroke related problems you (or your carers) are experiencing. This will help us to get the right help for you. The same screening is done around 6 months after your stroke to review your progress.
The Community Stroke Nurse Co-ordinating Team will see all patients with a new diagnosis of stroke. We do take telephone referrals. Patients and family members can contact the service directly.
We aim to see everyone who is referred to us as soon as possible.
We are not an emergency service. If you or someone you know is experiencing the symptoms of a stroke, ring 999 immediately.
