Community Stroke Team
On referral to the Stroke Community Service you will have a specialist Stroke assessment with the Senior Stroke Nurse or Associate practitioner dependent on location/postcode of home address. Additional advice, observation, screening and support relating to a Stroke diagnosis will be included.
For patients who require ongoing postural, balance and mobility assessment or mechanical/central post Stoke pain or upper limb management then a referral will be made to the Specialist Stroke Physiotherapist.
For patients who require a more intensive cognitive and psychological assessment or support, upper limb management and advice returning to work a referral will be made to the Specialist Stroke Occupational Therapist.
- Patients aged 18 or over
- Registered with an East Riding GP
- Have had a confirmed stroke - either a cerebral infarction or intracranial haemorrhage
- You have appropriate rehabilitation goals - this can include maintenance of condition.
