Community Stroke Team

Hull & East Riding Community Stroke Service

Following your referral to the Hull & East Riding Community Stroke Service, you will have an appointment with a member of the team who will go through a screening questionnaire with you. This will give us information about the stroke related problems you (or your carers) are experiencing. This will help us to get the right help for you. The same screening is done at 6 months to review your progress.

Referral Criteria Hull:

  • The client must be registered with NHS Hull GP.
  • The client must require therapy input due to a problem resulting from a stroke.
  • The client must require therapy input from one or more of the following:
  • Occupational Therapy, Physiotherapy, Speech and Language Therapy, Clinical Psychology or nursing.
  • The client must be over 18 years of age.
  • Referrals will be accepted from any health or social care professional using the designated referral form.
  • Common reasons for referral are difficulties with the following:

Mobility, cognition, perception, speech, swallowing, communication, social integration, psychological problems, difficulties with activities of daily living or personal care. However, this list is not exhaustive.

NB:

The Community Stroke Nurse Co-ordinating Team will see all patients with a diagnosis of stroke and will accept telephone referrals. Patients already known to the service are also able to contact the service directly.

We aim to see all referrals within the timescales specified below.  However, due to staffing levels, this may not always be possible.

  • Urgent swallowing issues to be seen within two working days.
  • Those at immediate health and/or social risk will be seen within 5 working days.  

Examples of this would include skin integrity, aspiration, chest infection, carer breakdown, cognitive or perceptual issues impacting on safety.

  • Patients who are less than 6 months post stroke will be contacted and seen within 72 hours from receipt of referral
  • Patients discharged from an inpatient setting will be contacted within 72 hrs from receipt of referral.
  • All others to be seen within 10 working days.

  • The client must require therapy input due to a problem resulting from a stroke.
  • The client must require therapy input from one or more of the following: 

Occupational Therapy, Physiotherapy or nursing.

  • The client must be over 18 years of age.
  • Referrals will be accepted from any health or social care professional using the designated referral form.
  • Common reasons for referral are difficulties with the following:

Mobility, cognition, perception, social integration, difficulties with activities of daily living or personal care. However, this list is not exhaustive.
