Heart Failure
Our Heart Failure Specialist Nurse service provides support, advice, medication and reviews for patients diagnosed with heart failure.
We help improve your quality of life and reduce the risk of you being re-admitted to hospital. Our aim is to help you do the things that matter to you, stay as independent as possible, and prevent problems getting worse where we can.
How we support you
When you have been referred to our service, a specialist nurse will carry out a full assessment of your condition. This will include asking questions about your past and current health, and what tablets or medication you may be taking.
Appointments are available across Hull and East Riding. If you are housebound, you can be seen at home.
Support may include:
- Reviewing your prescribed medication
- Co-ordinating your care
- Education and advice for patients, carers, families and other health professionals
The nurse may need to change medication and will explain this fully to you. Your blood pressure, pulse and weight will be recorded and blood tests may be taken depending on your condition and/or following changes to medication.
We will encourage you to monitor and manage your condition where possible to the point where you are stable and have a plan of care for your doctor to follow.
What happens after I am discharged?
After discharge, you may contact the team with any questions relating to your heart failure
What to expect
- You can be referred by a healthcare professional, such as your GP.
- Once we receive your referral, the team will review it (this is called triage) to decide who is best to support you.
- We will normally send you a text message or email letting you know when your appointment is, or if you need a different form of communication, we can call or send you.
- If you have previously received support from the service and your condition becomes unstable, you or a family member can refer you back into the service.
Inclusion Criteria
You can use this service if you:
- Are 18 years or older
- Are registered with a Hull or East Riding GP
- Have evidence of heart failure that meets the service criteria
- Patients who do not meet the criteria, but who have a heart failure diagnosis, may be considered after discussion by the team.
