Hull and East Riding Community Rehabilitation
Our Community Rehabilitation service consists of Physiotherapists, Occupational Therapists and Associate Practitioners.
After someone is referred to the service, the team will complete a triage and carry out an initial patient assessment to determine risk factors, patient needs and limitations. The team will then provide rehabilitation within the patient’s own home, to work on specific goals such as progression mobility, transfers and independence.
Referral Criteria
- Patients aged 18 or older registered with a Hull or East Riding GP
- Patients who are currently housebound and not able to access transport to other community services.
Exclusion Criteria
- Under 18 years old
- MSK Physiotherapy assessment that someone can attend a clinic, refer to MSK services.
- Presenting with difficulties which are due to a learning disability that cannot be managed in mainstream services, Refer to the CTLD Team
- For equipment provision only, refer to Social Services Occupational Therapy for provision of splints/orthotics only
Operating hours
9am to 5pm Monday to Friday.
