Hull and East Riding Community Rehabilitation

Our Community Rehabilitaion service provides Physiotherapy and Occupational Therapy assessment and treatment of patients who have a rehabilitation need due to a physical or neurological condition.

We work with people who have long term conditions, illness or injury, and aims to optimise people’s function and independence, and to prevent deterioration where possible. Our aim is to help you do the things that matter to you, stay as independent as possible, and prevent problems getting worse where we can.

 

How we support you

You may be supported by a Physiotherapist, an Occupational Therapist, an Associate Practitioner, or a combination of the three. They will take time to talk with you about your situation and carry out an assessment to understand your needs.

Support may include:

  • Advice and guidance to help you manage your condition yourself
  • Rehabilitation exercises or programmes
  • Support to build confidence and skills for everyday activities, such as moving around safely or managing daily tasks

Where appropriate, we also work with your family or carers to help you achieve your goals and take part in everyday life.

 

What to expect
  • You can refer yourself online here, or you may be referred by another healthcare professional, such as your GP.
  • Once we receive your referral, the team will review it (this is called triage) to decide who is best to support you.
  • Your first assessment is usually by phone. We will send you a text message or email with a booking link, so you can choose an appointment time that suits you.
  • After your assessment, rehabilitation may take place in a clinic or in your own home, depending on your needs and goals, such as progression in mobility, transfers and independence.

 

Inclusion Criteria

You can use this service if you:

  • Are 18 years or older
  • Are registered with a Hull or East Riding GP
  • Are housebound and unable to travel to other community services
  • Have a rehabilitation need due to a physical or neurological condition

 

Exclusion Criteria

This service is not suitable if you:

  • Need a musculoskeletal (MSK) physiotherapy appointment that you can attend at a clinic (please refer to MSK services instead)
  • Have difficulties related to a learning disability that cannot be supported in mainstream services (please refer to your GP)
  • Need equipment only (such as splints or orthotics) – please refer to Social Services Occupational Therapy

 

Operating hours

9am to 5pm Monday to Friday.
