Intermediate Care Community Team

This team provides short term intermediate care for people in their own homes or in residential care. This may be after a hospital stay, rehabilitation in a bedded unit or support at home for recovery from an injury or illness. The team is made up of physiotherapists, occupational therapists, associate practitioners, community nurses and care staff.

Together we will plan a programme of rehabilitation with you to help support your recovery, health and wellbeing.

What can you expect from us?

 We will assess your individual therapy, nursing and care needs, considering your previous routines, levels of independence and mobility. We will then look at what you find difficult and what your strengths are, to help you to identify what an achievable goal is for you. Even things like walking to the toilet and helping with your own personal care can help with your rehabilitation. Every movement counts!

What we expect from you:

  • To be willing to join in with your planned rehabilitation
  • To tell us if you have any questions or concerns
  • To manage your own rehabilitation between sessions by doing the exercises and activities we give you
  • To follow the professional advice and use the aids and equipment we give you
  • To take part in the discharge planning process if you can
  • To work with us to achieve your rehabilitation goals with mutual respect and compliance
  • Not to take photographs or recordings of staff or other patients.

How do we know your rehabilitation is working for you?

We hope that you will see an improvement in what you are able to do while you are with us. The therapy team will carry out reviews with you to see how far you have progressed.

What happens if you can’t meet the planned goals?

If you can’t meet your planned goals, we will review them with you and identify other things that may help you. It is important to understand that you may not be able to do everything in the same way as you did before you were admitted to hospital. Our job is to help you find ways to adjust to this.

What happens if you choose to not join in with your rehabilitation?

There may be times where you do not feel well enough to do your therapy. We understand this and will support you. However, if you choose not to take part in your therapy on a regular basis and without a good reason, you may be discharged from the service early. We will give you a programme and advice to help maintain the level that you are at before we discharge you. If you choose not to follow our advice, you will be asked to sign a disclaimer form. This will confirm that you understand the risks you are taking by choosing not to follow professional advice.
