The Team
The Cardiac Rehabilitation Service is s a multidisciplinary team of healthcare professionals who work together to help you recover safely after a Cardiac event.
Cardiac Rehabilitation Nursing Team:
Your first appointment with the Cardiac Rehabilitation Service will be with a member of the Specialist nursing team. During this appointment we will carry out an initial assessment to discuss your diagnosis, determine risk factors, identify your needs and limitations and provide education on medications and heart health. This assessment will last approximately 60 minutes.
If deemed appropriate, we may also contact you again by telephone to review your symptoms and discuss your follow-up appointments.
At the end of your Cardiac Rehabilitation programme, you will be offered a reassessment with a member of the Specialist Nursing Team. During this appointment, we will review your symptoms and the goals agreed at your initial assessment.
- Cardiac Rehabilitation Specialist Physiotherapist:
The physio team will carry out an assessment to determine your current level of fitness and will then design an exercise program specific to your needs and goals.
- Exercise Physiologists / Specialists
Guide supervised workout sessions and track your exertion levels during physical recovery.
- Cardiac Rehab Physio Assistants
Support clinical assessments, help during exercise classes, and carry out your reassessment at the end of your exercise programme. They are also able to refer you on for further exercise programmes if appropriate.
- Admin Assistants
Handle patient referrals, appointment bookings, and communication.
The Specialist Nurses and Physiotherapists can refer you to a range of other services. Examples of the services available include Dietitians, Occupational Therapists, Emotional Wellbeing Services, and Erectile Dysfunction Services.
These referrals will be discussed with you during your appointment and made where clinically appropriate.
