Holy Name Social Hub
The Holy Name Social Hub is run by experienced health and care professionals as part of the CHCP family of services, offering high quality day activities in a friendly and inclusive space for people aged 18 and over.
Our friendly team of health and social care staff are dedicated to helping you stay active, connected and independent in a safe and welcoming environment.
Our activities range from gentle exercise and creative sessions to social events and personal care support. We tailor every experience to your interests, routines and personal goals. Your wellbeing is at the heart of everything we do.
We work closely with you to develop a personalised plan, ensuring you enjoy meaningful activities and maintain your independence while feeling part of a vibrant community.
Our service brings together social activities, community engagement, wellbeing support, and personalised care, so whether you wish to make new friends, try something different, or simply enjoy a positive and supportive atmosphere, we’re here to help you get the most out of every day.
Who is the Social Hub for?
We support:
- Older people who may be isolated or need daytime support
- Adults with physical or mild cognitive impairments
- People looking for social connection, structure and wellbeing
- Carers who need respite during the day
You can access the service through the Hull City Council Social Care team or you can contact us directly.
How often can I attend?
- You can come to the Social Hub every day or just one for day or a few days a week; whatever works for you.
- We will review your plan regularly to make sure you are getting what you need.
Funding and costs
- Funding may be available through Hull City Council Adult Social Care following an assessment of your needs. If you do not qualify for local authority funding, you can still come to the Social Hub but there will be a cost.
- Our team can guide you through the referral and funding process and we will always provide clear information about costs before you start.
