What is the Holy Name Social Hub?
Holy Name Social Hub is a daytime support and activity service where people can enjoy social interaction, structured activities, wellbeing sessions and community involvement in a safe, friendly environment. This type of service is often called ‘Adult Day Services’.
Who can attend?
The service is for people aged 18 and over who would benefit from:
- Social engagement
- Support during the daytime
- Structured activities
- Maintaining or improving independence
We support older adults, people with low-level physical support needs, mild cognitive changes or those experiencing isolation.
Do I need a referral?
You can be referred by:
- A GP or health/social care professional
- A family member or carer (with consent)
- Or you can refer yourself and pay to attend.
Is this a free service?
No.
Holy Name Social Hub is a paid service.
You may receive funding through Hull City Council following a financial assessment or you may choose to pay for the service yourself.
We will always explain all costs clearly before you start.
How much does it cost?
The cost depends on:
- Whether you receive social care funding
- Whether you choose to pay to attend
- Optional lunch for morning or afternoon sessions
We will provide a clear breakdown of costs during your assessment to help you to make decisions about the care you would like.
Can I try the service before I decide?
Yes.
We offer a free taster session to help people decide whether the Social Hub is right for them. You will need to make an appointment with the team for a taster session.
What activities are available?
There is a varied timetable and we are happy to explore introducing new activities if clients request them.
Activities may include:
- Arts and crafts
- Music, singing, dance and movement
- Quizzes, puzzles, games
- Chair based exercise and wellbeing sessions
- Community outings
- Gardening and sensory activities
- Visits by performers and themed events
Do you offer community activities?
We aim to provide at least 2 hours of community-based activity a day, where this is appropriate. These could include:
- Walks
- Visits to local venues
- Guest sessions
- Intergenerational activities
- Community events
What does a typical day look like?
A day at the Social Hub may include:
- 9am – Arrival, welcome, refreshments
- Morning activities (creative, social or wellbeing)
- Hot dinner
- Afternoon activities (community engagement, group sessions, games)
- 3pm – End of day
Do you provide meals?
Yes.
We will give you a hot dinner and we offer drinks and snacks throughout the day.
Is transport available?
Holy Name Social Hub does not offer dedicated transport but you may be able to use Community Transport depending on:
- Whether you're eligible
- If local transport providers are available.
We are happy to help you explore travel options during your assessment.
What support can staff provide?
Our trained staff can help with:
- Personal care needs (where agreed)
- Mobility and general supervision
- Social and emotional support
- Safe medication prompts (if part of your plan)
- Activity participation
Do you support people with dementia?
Yes.
CHCP is a Dementia Friendly organisation and Holy Name also supports people with dementia who live in the care home.
We offer dementia friendly activities and structure as well as signposting and support to access other services that can support people with dementia.
How often can I come to the Social Hub?
You can choose:
- One or several days a week
- Short or long term attendance
Your plan will be reviewed regularly to ensure it still meets your needs.
What should I bring?
We recommend:
- Comfortable clothing
- Any medication you may need during the day (clearly labelled)
- Mobility aids like a Zimmer frame, if you need them
- A packed lunch if you don't want to buy a meal
What if I can't come on my usual day?
Please notify us as soon as possible.
We understand that appointments and unexpected issues happen but we do need to know if you aren't able to come.
Can family or carers visit?
Yes. Visits can be arranged by appointment.
This helps us to keep a calm and safe environment for everyone.
How do I give feedback or make a complaint?
What happens if I want to stop attending?
You are free to stop at any time.
We will:
- Discuss your needs
- Support a smooth transition
- Give you information about alternative services if you need it
