How it works

1. Referral 

You can be referred through: 

• Hull City Council Adult Social Care Accessing adult care – Live Well Hull 

• Family, carer or by self-referral (privately funded) 

2. Assessment 

You will have an assessment to make sure the Holy Name Social Hub is right for you  

3. Taster session

Before you decide to join the Social Hub, you can come to a taster session to help you get a feel for it and meet the team. 

4. Starting your sessions 

We develop a personalised plan so your time with us reflects your needs, preferences and goals. 

Funding and costs 

Holy Name Social Hub is a paid service. 
 
You may be eligible for funding through Hull City Council Adult Social Care, subject to a financial assessment. Click on this link to find out more: Hull City Council Social Care team  

If you do not meet the criteria for local authority funding, you can pay to attend. We will always explain costs clearly before your sessions begin.
