Urgent Community Response

Community based service provided by a multi-skilled team to patients who need a response within two hours for a very urgent care need. The two-hour response aims to prevent hospital admission.

Inclusion criteria:

• A patient at risk of an acute hospital admission?

• Registered with a Hull or East Riding GP

• Aged 18 or over

• Response needed within 2 hours to avoid a hospital admission.

Exclusion criteria:

• Patient requires immediate emergency response e.g. life threatening, sepsis

• Mental health crisis.
