We collect data to make sure the services we provide are giving good care. We will use your data to see when you have gone into or visited one of the local hospitals in this area.

What’s new?

Working with InHealthCare and YHCR to subscribe you to the YHCR. This will tell us when you have gone into hospital and when you have left. So we can support you.

Information we collect

Information we collect will be used to help you directly. We will also use this information to make sure we can continue to make our services better.

Reporting

Any reporting done will not be able to identify you as you. We may report on ages and where our patients are from, but will not be able to tell that person is you. This means it is anonymous.

The information a service needs

Services will have to record the following information:
Name, NHS Number, Gender/Sex, Date of Birth, Email Address and Postcode.
This is so we know that you are you.

Yorkshire Humber Care Record

Once we know that you are you, and you are happy for us to use your data. We will subscribe you to the YHCR. You can unsubscribe at any time, just let us know.

If you would like to see the data, we hold about you, please email our Access to Records team at CHCP.AccessToRecords@nhs.net. We will try to get this to you as quickly as we can.

Who will your information be shared with?

We will share your information with your doctor and other healthcare services that care for you.

Confidentiality

We will not give your information to any one we haven’t mentioned without your permission, we would always ask.

Legal Duty

We may have to give your information to someone else if a serious crime has been committed, if there is a risk to the public or we have a legal duty to pass on information. This keeps you safe.

What to do if you wish to be removed?

If you wish to be removed from the YHCR service please contact Care Co Ordination by email chcp.customercare@nhs.net and this can be done.

How long will your information be held?

We store information for the time stated in Records Management Code of Practice for Health and Social Care 2021.

We will always do our best to make sure that your information is always as safe as possible. Using the right policies, procedures, and up-to-date technology to help us.
