Intermediate Care

Our intermediate care service provides a short-term rehabilitation service after illness or injury.

Health and social care professionals work together to support you if you are suddenly unable to safely manage everyday activities. This might include physiotherapy, occupational therapy, community nursing and help with personal care, depending on what you need.

We will assess your therapy, nursing and care needs, looking at your previous routines, how independent you are and how mobile you are. Then we will look at what you find difficult and what your strengths are, to help you to decide what an achievable goal would be for you. Please see the rehab contract below, which will support you during your time with the service.

Everyone involved in your care will go to team meetings to keep up to date on your progress. Our care co-ordination team will oversee your care and monitor your progress towards your goals.

Intermediate care can take place:

  • In your own home
  • In a community bed (in one of our rehabilitation centres)
  • In a mix of both, depending on what suits you best

How long does it last?

  • Intermediate care treatment usually lasts up to six weeks
  • It may be shorter if you meet your goals sooner
  • If you need longer term rehabilitation, you may be referred to community rehabilitation services 

 How will intermediate care help you?

  • We help you to do things for yourself again like washing, dressing, cooking, or getting around
  • We help you get better after a stay in hospital or an illness
  • We help you to stay at home, rather than go into long-term care
  • We support you to build confidence so you may need less support in the future.

Who is intermediate care for?

Intermediate care is for people aged 18 and over who have a condition that is medically stable and can be managed safely in the community. Please note: referrals must come from a health or social care professional.
