
Intermediate Care
Our Intermediate Care service provide a short-term rehabilitation service after illness or injury.
Health and social care professionals work together to support you in situations where you are suddenly unable to safely manage everyday activities due to illness or injury. This includes physiotherapy, occupational therapy, district nursing, community nursing and assistance with personal care as needed.
We will work with you to make realistic rehabilitation goals, providing a range of services to help you achieve them.
All professionals involved in your care attend weekly team meetings to keep up-to-date on your progress. Care co-ordination will oversee your care and monitor your progress towards your goals.
Intermediate Care can be delivered:
- In a person’s own home
- In a community bed (within one of our rehabilitation centres)
- Or a mix of both, depending on what is most appropriate for each person
How long does it last?
- The full Intermediate Care pathway usually lasts up to 6 weeks
- It may be shorterif the person meets their goals sooner
- If someone has longer-term rehabilitation needs, they can be referred to community rehabilitation services for ongoing support
What is the goal of Intermediate Care?
- Help people do things for themselves again — like washing, dressing, cooking, or getting around
- Support recovery after a stay in hospital or an illness
- Help people stay at home, rather than go into long-term care
- Build confidenceand reduce the need for support in the future
Who can access Intermediate Care:
- The service is available to people aged 18 and above,
- who have a condition that is medically stable,
- and can be managed safely in the community.
