Macmillan Community Nurses (East Riding)

We provide palliative and end of life care and support to all patients who have a life limiting disease in the community. The service includes all palliative and end of life care patients regardless of diagnosis.

We work with other health care professionals, social services, statutory, voluntary and private agencies to assess patients' needs and to provide patient focused, responsive, timely and sensitively-delivered palliative and end of life care.

Referrals are accepted from patients, carers and all health and social care professionals through our Care Co-ordination Centre on 01482 247111.

Referral Criteria:

A palliative diagnosis
Aged 18 or over
Patients requiring specialist symptom control and management.
