CHCP Diabetes Service

Our Specialist Diabetes Service works closely with individuals, local GP surgeries, community nursing teams, and nursing and care homes within Bridlington, Hornsea, Driffield, Leven and Beeford.

How we support you

We provide education and support to understand, and manage diabetes diagnosis, healthy glucose management, and the variety of medications (both oral and injectables) used to treat diabetes. In addition, we also educate people about lifestyle changes to improve general health and day-to-day wellbeing and deliver regular “Living with Diabetes” courses in the area; which your GP can refer you to if appropriate.

Inclusion Criteria

You can use this service if you:

  • Are 18 years or older
  • Are registered with a GP in Bridlington, Hornsea, Driffield, Leven and Beeford
  • Require support with:
    • Insulin initiation and any insulin changes (dose adjustments or changes to insulin regimes)
    • Modifying or reviewing diabetes medications and/or insulin regimes
    • GLP-1 initiation and ongoing management
    • Support with technology used in diabetes care, including CGM (continuous glucose monitoring)
    • Hospital discharge after starting insulin, hypoglycemia and DKA (diabetic ketoacidosis)
  • Patients on triple oral therapy whose diabetes control remains poor.
Exclusion Criteria

This service is not suitable if you:

  • Are under 18
  • Use an insulin pump
  • Have gestational diabetes
