At East Rising Community Hospital bedded unit, we provide intermediate care, stroke rehabilitation, palliative and end of life care services.
This page is about our short-term rehabilitation or reablement service for people who are at risk of going into hospital or who have been in hospital because of illness or injury.
We help you to recover and increase your independence so that you can either go home, return to your care home, or go to a new one, depending on your health.
At the start of your stay, you will have some initial assessments to help the therapists looking after you to set some goals and timescales for your recovery.
They will write a care plan that everyone involved in your care will follow, so that therapy will be part of all your daily activities, encouraging you to do them by yourself wherever possible.
During your stay, your goals and progress will be reviewed regularly and we will look at what you may need in order to go home.
This might be equipment, adaptations and/or referrals for ongoing support or continued intermediate care. A lot depends on you taking part in your rehabilitation and how much progress you make.
When you’re ready to leave, we will work with all the people involved in your care to make sure your discharge goes smoothly.
Prescriptions
If you normally pay for prescriptions and a prescription is issued whilst you are staying with us, you will need to pay for your prescription as you would at home.
