Health and Social Care Team

This service is made up of health care assistants, who are skilled in the delivery of high quality specialist palliative nursing care and support in a patient's own home. They help with personal care and comfort, provide psychological and emotional support to patients and their carers and prepare light meals and help to provide respite breaks for carers.
