
Macmillan Community Therapy
The Macmillan Community Therapy Service work with patients to maximise independence, productivity and wellbeing, enabling patients to live actively until death.
The service is made up of physiotherapists and occupational therapists, offering interventions such as fatigue management, relaxation and mindfulness activities, providing equipment and offering fast-track referral for major adaptations such as stairlifts. They create individual exercise plans to maxmise and retain mobility and offer advice about breathlessness and breathing techniques.
The service includes all palliative and end of life patients, regardless of their diagnosis.
Referral Criteria
- Patients aged 18 or over
- Registered with a Hull GP
- Patients who have a palliative diagnosis and who need specialist therapeutic intervention.
Referrals can be made by patients or healthcare professionals.
