Palliative Care Night Team

We are a team of health care sssistants skilled delivering high quality end of life nursing care and support in patients' own homes. The team stay in the patient's home from 10pm till 7am to ensure the patient is cared for, allowing carers and loved ones the chance to sleep or go home.

Referrals are accepted from health and social care professionals for patients who meet the following criteria:
