Palliative Care Night Team
What is the Service?
The team are all Health Care Assistants skilled in the delivery of high quality end of life nursing care and support, within the patient’s own home. The team stay in the patients home from 10pm till 7am.
Aim of Service
The service aims to ensure the patient in cared for in the overnight period, allowing carers and loved ones the chance to sleep or go home.
Service Referral
Referrals are accepted from health and social care professionals and can be made through the CHCP CIC Single Point of Access Specialist Palliative Care Services Tel: 01482 247111.
Referrals will only be accepted by patients who meet the following criteria:
- Patients must be over 18 years old
- Registered with a Hull GP
- Have a medically identified palliative care need.
Expected Outcomes for the Service
To ensure that patients with a life limiting illness receive the highest possible level of individualised care within their own home, and allow patients to be cared for in their preferred place of care.
