Community Heart Failure Specialist Nursing

The Community Heart Failure Specialist Nurse Service provides support, advice, medication and clinical reviews for patients diagnosed with heart failure. The service is made up of senior nurses with specialist knowledge and skills in cardiology and heart failure.

How to access the service

Access is through a GP or health professional although patients who have previously received support from the service can also re-refer themselves or have a family member re-refer them if their condition becomes unstable. Referrals are via a health professional and should be made by email to chcp.247111@nhs.net 

Referral Criteria:

  • Over 18 years of age.
  • Registered with a Hull or East Riding GP.
  • Evidence of heart failure with reduced ejection fraction (HFrEF) on echocardiogram, cardiac MRI or coronary angiogram. This is an ejection fraction of 40% or below.
  • Patients who need optimisation of treatment for HFrEF to prevent decompensation.
  • Patients who do not meet the above criteria but who have a heart failure diagnosis with an aetiology other than HFrEF may be  considered after peer discussion within the service team.
