Evolve Service User Experience Group

Are you a former patient or carer of someone who has experienced an eating disorder and attended Evolve in the last 18 months?

If so please join our Evolve Service User Experience Group and find out more about how you can get involved in improving services for people with eating disorders in Hull.

We need you, for your experience, interest and knowledge to:

Give ideas and suggestions on how our service could be improved
Share ‘good practice’ and what we do well
Help the service to plan and make decisions about the treatment it provides
Bring an independent user perspective to the design, improvement and delivery of the service.

How to access the service:

Referring yourself, a friend or family member: You can refer online by clicking here.

You can also be referred into the service by a health professional.

If you don't have an NHS or SystmOnline log-in, you can just click on the self-referral option, underneath the Medical Request heading.

Opening times:

Monday to Friday, 8am - 6pm (reception open: 9am - 5pm)
