Hull & East Riding Falls Service
The Hull and East Riding Falls Team is a multi-disciplinary team consisting of nurses, physiotherapists, occupational therapists and rehabilitation assistants.
The service provides falls prevention to the Hull & East Riding population. The team’s aim is to minimise risk and prevent further falls where possible, whilst maintaining the independence of those who have a history of falls.
Fracture Liaison Service (Hull only) will look at those patients who have fractured (broken a bone) and check for history/risk of osteoporosis.
We work with patients to help them find ways of doing things that reduce the risk of falls including making simple changes to their home, provision of aids and adaptations, helping with exercises to improve strength and balance or reviewing medications.
- Patients aged 18 or over
- Registered with a Hull & ER GP (not Pocklington GP)
- Had one or more falls within the last 6 months
- Alcohol dependence
- No falls
- Pocklington GP
Referrals can be made through the Care Co-ordination Centre, and can either be self-referrals or through any health professional.
